The problem is that there are bogus operations known for passing themselves off as something they are not. With the world we live in, I think people should be cautious with alleged surveys.

No message left, No legitimate reason for calling. CID name is a variant the number that is being called from "18019218152"

Worth noting carrier is  "Level3 VoIP in Provo, UT" At least its not another scammer using magicjack/ymax.

Called my cell phone, and I didn't answer. Phone rang two or three times and then they hung up. No idea why a number from Utah would be calling me.

Just received a call from this number.  They called 10/2/14 at 1:55 p.m.  I did not answer and they did not leave a message.  My phone shows the call as coming from Provo Utah.  I am located in Ohio.  I am trying to recall if I'd recently "signed up" for anything online lately that might have sold my number, but nothing is coming to mind.
