"Anna" of Senior Care, had my name and address, asked if I was on Medicare and did I still knee or back pain; said company was based in Florida; would not provide company's main telephone number or address, instead hung up on me.

Medicare PHISHING scam of some sort. I'm in my 30's! Number blocked.

Call came in on 12/05/16, like many of these posts.  Call came in via Call waiting while I was on a LEGIT Medical call!  Got the # via *69 after.  No way to block under these circumstances. :(  Have to wait until they try & scam me again.

Also, we have family in that area with 646 numbers.  Always a moment of concern wondering if its one of them calling in an 'emergency' with a different number.
