Caller ID said "Care ImprovementPlus", a Medicare plan. After 9 PM, Saturday; eight rings. United Healthcare, largest Medicare/Medicaid provider, has its HQ in Minneapolis, but so does the largest US debt collection agency, Allied Interstate. Both sail under many different flags.

This is a phone number that is linked to the Care Improvement Plus insurance coverage connected to Medicare.

I have gotten several calls from this number, some have no reply to answer,only silent line.  I recently cancelled my association with Care Improvement Plus Insurance. (from comments on site)
