We received this SCAM also. There is no contact information and it looks just like an invoice. This is about as questionable and low marketing approach that I have ever witnessed. Yes....In some fancy font at the bottom of the page that is nearly illegible,  they state that the paper is not a bill or invoice....but someone could be easily fooled if they weren't paying attention. Shameful!

My Church received a fax from them. I was just a coincidence that it came in at a time that I was proposing some upgrades to the network. It is a scam from someone that is sending out faxes. Anyone using a yahoo, gmail or other free email account for their business should be considered suspect.

On March 25, 2012, I received a fax with the same information - a document labeled "Proposal" with a total due amount of $190.00.  This document could be inadvertently paid as a valid invoice (I am guessing that that is exactly what the sender wants to happen).

I looked up the address.  It is a Post Office in Oklahoma City, OK, so I am guessing that the # 1475 in the address is a post office box.

For this reason I filed a complaint with the US Postal Inspection Service.  I encourage everyone else to do the same.  Even though the sender is using a fax machine to send these fake bills, any money sent goes to a US Post Office Box.  This could constitute some type of mail fraud.  Maybe the Post Office can put a stop to this crap.  File your online complaint at: https://postalinspectors.uspis.gov/forms/MailFraudComplaint.aspx.
