It is Icon Office Technologies.  They claim to be a telephone solicitor of office supplies.  There are a number of complaints lodged against them on the Better Business Bureau's website that complain this company ships and bills for toner cartridges that were never ordered.

Their fax number is (323) 544-9055.  The address they use is 17100-B Bear Valley Road, Suite 191, Victorville, California 92395.  However, this address was investigated and it is not an office address, it is the address to "Postal Annex" which is a building that maintains mail boxes and ships items out.
