I called it back, and it's a survey for several differnt medical facilities. So I assume if you had a doctor's appointment recently, they are calling to have you do a survey about their preformance.

They called about taking a survey. They had my name and medical clinic that I attend, however the doctor's names were wrong and I had never heard of the two doctors named. She said she would send my survey back due to incorrect info.
