I have also received several calls from this number and decided today to call them back since they haven't left any voicemails.  Like others, I got the automated message that they called because of a website of their affiliates that I visited and as a reward I had a $50 Wallmart giftcard waiting for me if I took a 3 minute survey of "online shopping habits".  I did take the survey and instead did a search online for this number which led me here.  
I am surprised to find so many others getting the same call, all in the chicago area, and all on their cell phones, not land lines and that they would ask for you to pay shipping, with a credit card no less.  I am also in the chicago area and Im wondering if there is any connection between the cell phones called.  I have T-mobile and I'm wondering if all or some of you getting these calls also have T-mobile?  
I have a strong feeling this company is up to no go, especially since I, like others on here did NOT give my number out to ANY website or written survey/forum etc.  I am very carefully about who I give my cell phone number to, and if its not someone I know well I give them my landline number.

I got multiple calls from that number being on do not call list as well.
I guess the only FAST way to shotdown those boneheads is to feed them with phone numbers of Governent officials. Trust me, next day after boneheads call officials, they will be out of business.
No forms will require to file with FTC. Just one phone call from Senator/Representative offoce to FTC, and the end for that bloodsucking business.
